Cradle of Love LLC has an exciting new opportunity for a Housing Stabilization Services Coordinator!
The purpose of the Housing Stabilization Coordinator is to manage the Housing Stabilization program, ensure compliance with licensing requirements, track and oversee billable hours, support, and manage employee performance, and provide ongoing supervision to Housing Consultation Assessors and Housing Stabilization and Transition Specialist in the delivery of housing stabilization services in the community.
Full time, exempt position. Hours typically between 8:00am and 5:00 pm Monday-Friday.
Job duties include:
Housing Stabilization Services Program Management-
Ensure the Housing Stabilization Service program operates with efficiency, sustainability and profitability while aligning with organizational goals and licensing requirements. Develops programmatic goals, quality control procedures, and a program budget. Continually seeking new referral sources to increase services offered to potential participants. Maintain general knowledge of third party insurance coverage and benefits by having a working understanding of medical billing.
Staff Supervision and Management-
Hire the highest quality of staff who support the mission, vision, and values of the organization. Manage performance and provide coaching to staff through case consultations and enforcement of agency guidelines, policies and procedures. Provide direction and support to the Housing Stabilization staff by conducting field observations. Review progress notes of onsite visits to ensure they are accurate and consistent with the purpose of the visit and goals of the treatment plan.
Housing Stabilization Services
Provide BOTH Housing Consultation Assessment and Housing Stabilization and Transition Services to help people with disabilities and seniors find and keep housing by developing a person centered housing support plan with the individual and their stakeholders. Model services practices that demonstrate respect for the participants, and promote collaboration and coordination. Maintain ongoing, updated participant’s files to include updated plans based on state guidelines.
Housing Consultation Assessments
Provide assessment and assistance in developing a housing focused person-centered plan, assist the person to access needed state plan services that support housing stability and provide referrals or information about other needed services.
Housing Transition Services
Provide services that assist a person to plan for, find and move to a home in the community.
Housing Sustaining Services
Housing sustaining services support a person to maintain living in their home in the community.
Maintain ongoing, updated client files to include contact documentation and updated housing focused person-centered plan. Plans are updated on an annual basis or more frequently if the person requests a plan change, experiences a change in circumstances or wants to change housing stabilization providers.
Develop and maintain positive working relationships within the community.
This position requires the following knowledge and skills:
Understanding of Housing laws, tenant rights, basic contacts law, evictions, leases, and knowledge of housing resources.
Considerable knowledge of local housing resources and other human service resources.
Effective written and oral communication skills
Effective time management and organizational skills
Strong presentation and interpersonal skills
Ability to manage multiple priorities, work independently and take initiative
Ability to maintain a professional composure and effectively represent the organization
Ability to maintain a high level of confidentiality
Exercise good judgment and apply effective listening skills
Proficient in and knowledge of Microsoft applications and EMRS
Ability to operate standard office equipment independently
EDUCATION AND WORK EXPERIENCE
This position requires a Bachelor’s Degree in Human Services, Behavioral Science or related field.
Successfully completed Vulnerable Adults Mandated training and online Housing Consultation training and Housing Stabilization – Transition/Sustaining trainings from DHS.
Preferred experience: 1-2 years of supervisory experiences, experience working with homelessness and substance use disorders.
ADDITIONAL POSITION REQUIREMENTS
This position requires a valid driver’s license and proof of auto insurance as the employee will be required to drive their own vehicle to perform certain job responsibilities.
This position requires a clear DHS background check.
